What determines longevity, performance, and well-being in surgeons

Surgical training prepares you to care for others. But it does not always prepare you to protect your own physical capacity over time. The assumption most surgeons accept


Many surgeons enter the profession with an unspoken expectation:


That physical discomfort, fatigue, and even pain are part of the career. That over time, the body will begin to decline. That limitations are inevitable. But this is not necessarily true.


Awareness changes outcomes

If there is no awareness of what is happening to your body — physically, emotionally, and functionally — it becomes very difficult to change the outcome. What you perceive as “normal” often becomes what you accept. And what you accept tends to define the trajectory of your career. A different outcome requires a different level of awareness.


The three elements that define surgical performance

Surgical performance is not determined by technique alone. It is influenced by the interaction of three elements:


1. The task


  • The surgical procedure itself


2. The environment



  • The operating room


3. The individual


  • The surgeon

  • physical capacity

  • movement efficiency

  • adaptability

  • recovery ability

  • The missing link


Most discussions focus on the first two. But the third — the surgeon’s own movement system — is often overlooked.


The three elements that define surgical performance


The movement system is the integration of multiple systems, including:


  • neuromuscular control

  • fascial organization

  • circulatory dynamics

  • biomechanical coordination

  • nervous system regulation

  • stability and the ability to generate and sustain force, including isometric control


When these systems are well integrated, the body functions efficiently. When they are not, the body adapts.


Adaptation and long-term consequences


Surgical practice requires:


  • precision

  • stillness

  • repetition

  • sustained posture


Over time, the body adapts., At first, these adaptations support performance. But over time, they may begin to:


  • reduce efficiency

  • increase strain

  • limit recovery capacity


This is when symptoms begin to appear.


Individual variability and early onset


Not all surgeons respond to these demands in the same way. Each individual presents a different level of system capacity, adaptability, and resilience. Some tolerate the demands longer. Others begin to experience symptoms earlier — often during training.


It’s not just physical

As physical strain accumulates, it can also influence:


  • comfort during procedures

  • confidence in movement

  • overall sense of well-being


A proactive approach to longterm performance


Because these demands are cumulative, a reactive approach is not enough. The most effective strategy includes:


  • preparing the system before sustained demands

  • maintaining system efficiency during the career

  • periodically reassessing how the body is adapting
